Combines a FreeRegionMap and a TyCtxt.

This stuff is a bit convoluted and should be refactored, but as we transition to NLL, it'll all go away anyhow.

The context used for debug messages

free_regions: &'a FreeRegionMap<'tcx>

Free-region relationships.
